[osx] fixed, vda decoder under 10.6.4+. reserving 16 ref frames is silly. request kCVPixelFormatType_422YpCbCr8 image buffer format, PIX_FMT_BGRA to PIX_FMT_YUV420P is dog slow using sw_scaler.
